By checking this box, I am certifying the following: I will not attend in-person services if I experience the following symptoms of COVID-19: Fever (of 100 or greater) or chills; New Cough; Shortness of breath or difficulty breathing; Fatigue; Muscle or body aches; Headaches; New loss of taste or smell; Sore throat; Congestion or runny nose; Nausea or vomiting; Diarrhea. I have NOT recently been in contact with someone who exhibited these symptoms or anyone who has tested positive for COVID-19.

We're so excited to reconnect with you for in-person services!

All persons, even those included in the same family, should fill out the form individually.

Please bring lawn chairs, blankets, sunscreen, water, and most importantly, a mask for everyone in your party. We will have both services on the West Patio/West Lot.
There is accessible parking in the West Parking Lot. Please note in your sign up if you need to use the handicap parking section. All others, please parking in the East Lot.
Please consider signing up to assist with Sunday morning services. We have a variety of jobs from ushering to helping with the welcome table.
